If you are a former or current Berryhill, OK citizen, and you or a loved one have experienced illness as a result of the pollution in Berryhill, please submit a video of you sharing the following information.  Be sure to adhere to the technical specifications requested as well.

1. Use your phone to record yourself or another person telling your/their story.  Orient your phone vertically or in portrait mode as seen below:

2. Use one of the following scripts to record your short story about illness from the Compass Industries Landfill Superfund Site in Berryhill, OK:

Script for Self:
"My name is ___________ and I (currently live / lived (include dates and age range)), and I believe that I got (illness(es) from the Compass Industries Landfill (or other Berryhill source if you know of one - and share some details briefly) by (exposure route: air, water, etc.). Conclude by saying briefly how it has impacted your life."

Script for Other:
"My name is ___________ and I (currently live / lived (include dates and age range)), and I believe that (person's relationship to you and their first name) got (illness(es) from the Compass Industries Landfill (or other Berryhill source if you know of one - and share some details briefly) by (exposure route: air, water, etc.). Conclude by saying briefly how it has impacted your life."

3. Try to keep your video to one to two minutes in length, and no more than 100 mb in file size.

4. Submit your file at the following link where it says "Berryhill Testimonials" and put your full name, address, and phone number in the message field.  Sharing your video will be considered consent to share any or all of the video in the documentary, Berryhill.
